What do we do?
The Community Rehabilitation Service is
a team of health professionals that specialise in providing rehabilitation for improving
capabilities in the workplace or at
home.
This team can assess
your needs and assist you to regain or
maintain your role at work or your independence with activities of
daily living. We help you to achieve positive
rehabilitation outcomes.
Other options may include specialist advice,
applications to funding bodies for provision of housing modifications and
specialised equipment (for example, ACC, Enable, New Zealand Lotteries board),
home based rehabilitation programmes and onward
referrals.
We can provide:
- Contract services to ACC under the Social
Rehabilitation Contract
- Independent Assessment for Private Insurance
Companies working under the ACC legislation.
- Contract services to the Ministry of Health for
specialised assessments.
- Private contracts
with organisations that require our expertise.
- Return to Work - planning
and monitoring - Ergonomic
Workstation Assessment

- Workplace Training Workshops -
Out-of-Office Worksite Assessment

Who are you?
Client
You have been referred to the Community
Rehabilitation Service by your Case Manager for an assessment by a
health professional, to identify, with you, your needs and the
available options, to be able to be independent with activities of
your daily life. This assessment will usually take place in your
home. We will contact you.
ACC Case Manager
We
are a multi-disciplinary team, who can provide high quality assessments under
the ACC legislation. We are able to
provide both standard and complex social rehabilitation assessments, as
well as the following specialised assessments
- occupational therapy, physiotherapy, social work, specialised equipment, specialised
wheelchair seating, specialised nursing and housing modification
assessment services.
Independent Insurance Company Case
Manager
We are a multi-disciplinary team, who can provide
high quality assessments under the ACC legislation. We are able to
provide assessments and programmes tailored to meet your client's
individual requirements. Please contact us to discuss your
